The difference between sins and mistakes has to do with intent.

If someone purposes to insult and hurt a persons feels out of hatred that is a sin, but if with no malice a person says something without thinking what they were going to say and insults someone I would consider that a mistake.

I would hold that the mistake must be made right and if it is appropriate by asking for forgiveness of the person that was offended, the person that make it should take it very serious and look to the Lord to help them to be more sensitive and careful in their, and as it is a transgression against the perfect law of love the person should ask the Lord for forgiveness.
